Изучены современные представления о предраковых интраэпителиальных неоплазиях полового члена, изложенных в классификации ВОЗ опухолей мочевыделительной системы и мужских половых органов 2016 года. В 5 главе классификации 2016 г. «Опухоли полового члена» «Tumours of the penis» в разделе «Предраковые повреждения» (Precursor lesions) рассмотрены новые подходы к систематизации предраковых эпителиальных образований полового члена. В этой главе предложены две нозологические единицы: пенильная интраэпителиальная неоплазия (penile intraepithelial neoplasia), ассоциированная с ВПЧ-инфекцией, и дифференцированная пенильная интраэпителиальная неоплазия (differentiated penile intraepithelial neoplasia), не связанная с ВПЧ-инфекцией. В работе на основе личного клинического опыта по ведению пациентов с боуэноидным папулезом, лейкоплакией, лишаем склеротическим полового члена и баланитом псевдоэпителиоматозным кератотическим и слюдяным в ГВКГ им. Н. Н. Бурденко и литературных данных приведено соответствие этих клинических нозологических единиц различным гистопатологическим типам пенильных интраэпителиальных неоплазий. При боуэноидном папулезе выявляется пенильная интраэпителиальная неоплазия, ассоциированная с ВПЧ. Лейкоплакия, лишай склеротический и баланит псевдоэпителиоматозный кератотический и слюдяной являются клиническими диагнозами, при которым может выявляться дифференцированная пенильная интраэпителиальная неоплазия.

Modern vision of pre-cancious penile intraepithelial neoplasia contained in the 2016 WHO Classification of Tumors of the Urinary System and Male Genital Organs was studied. New approaches to systematization of pre-cancious epithelial tumors of penis were reviewed in the fifth chapter of the 2016 WHO Classification called «Tumours of the penis» in section Precursor lesions. In this chapter two nosological units such as penile intraepithelial neoplasia associated with HPV infection and differentiated penile intraepithelial neoplasia not associated with NPV were proposed. We analysed patients with papulosis bowenoid, leukoplakia, penile lichen sclerosus, pseudoepitheliomatous keratotic and micaceous balanitis in Main Military Clinical Hospital named after N.N. Burdenko in accordance with different histopathological types of penile intraepithelial neoplasia. Penile intraepithelial neoplasia associated with HPV is identified in cases of papulosis bowenoid. Differential penile intraepithelial neoplasia can be found in clinical dignosis of leukoplakia, penile lichen sclerosus, pseudoepitheliomatous keratotic and micaceous balanitis.
